Certification by gazetted officer

  • Officer’s name: Full name of the Gazetted officer or magistrate.
  • Designation: Official designation of the certifying officer.
  • Office address: Address of the certifying officer’s office.
  • Signature and seal: Signature and official seal of the Gazetted officer/magistrate.
