What is IMI?

Intensified Mission Indradhanush (IMI) is a government-led initiative launched in October 2017 to enhance immunization coverage in India's underserved regions. Targeting critical districts and urban areas, IMI aims to rapidly boost immunization rates, particularly among pregnant women and children up to five years old. It employs advocacy efforts to garner administrative and financial support, aiming for over 90% coverage in identified regions by December 2018. IMI integrates innovative strategies, including IT-based platforms, for sustained progress through routine immunization. By reaching the unreached and accelerating vaccine delivery, IMI plays a vital role in curbing preventable diseases and reducing child mortality nationwide.

Objectives of Intensified Mission Indradhanush

The Intensified Mission Indradhanush (IMI) aims to extend immunization coverage to underserved populations, accelerating the protection of pregnant women and children in identified critical districts. IMI's objectives include:

  • Achieving over 90% immunization coverage in identified districts and urban areas by December 2018.
  • Garnering administrative, political, and financial support through advocacy with relevant departments, ministries, and stakeholders to ensure full immunization for every child.
  • Ensuring that all children receive vaccines according to the national immunization schedule, focusing on those up to two years old and pregnant women. However, vaccination will be extended to children up to five years.
  • Sustaining IMI's progress by integrating routine immunization into IT-based platforms for continuous monitoring and planning.

Vaccines covered under IMI

IMI offers vaccination against twelve Vaccine-Preventable Diseases (VPD), including the following:

  • Diphtheria
  • Whooping cough
  • Tetanus
  • Polio
  • Tuberculosis
  • Hepatitis B
  • Meningitis and pneumonia
  • Haemophilus influenza type B infections
  • Japanese encephalitis (JE)
  • Rotavirus vaccine

Phases of Intensified Mission Indradhanush (IMI)

There are multiple phases of IMI which have been released over the years. Discussed below are these phases in brief:

  • IMI
    The government introduced IMI to immunize every unvaccinated pregnant woman and child under two years. This targeted drive aims to enhance immunization rates in specific cities and districts, targeting over 90% coverage by December 2018. IMI conducted four consecutive seven-day rounds from October 2017 to January 2018, prioritizing urban areas and high-risk districts. Selected areas, comprising 52 districts in eight North-Eastern states, 121 districts, and 17 cities in 16 states, were chosen based on comprehensive data analysis from national surveys, World Health Organisation monitoring, and Health Management Information System data.

  • IMI 2.0
    In 2020, the government initiated IMI 2.0 to extend vaccination coverage to unvaccinated and partially vaccinated children across 381 districts in 29 states/UTs. Completed by March 2020, the program received support from multiple ministries, including Urban Development, Youth Affairs, Women and Child Development, and Panchayati Raj, ensuring widespread vaccine accessibility.
  • IMI 3.0
    IMI 3.0 was launched to enhance routine immunization coverage, target unreached populations with available vaccines, and expedite coverage for pregnant women and children in specific blocks and districts from February to March 2021. It comprised two rounds, commencing on February 22 and March 22, 2021, across 250 pre-identified urban areas or districts spanning 29 states/UTs. Districts were categorised into 313 low-risk, 152 medium-risk, and 250 high-risk districts under IMI 3.0 guidelines, with a focus on pregnant women and children who missed vaccinations during the COVID-19 pandemic.

  • IMI 4.0
    In February 2022, the government initiated IMI 4.0, encompassing three rounds across 416 districts, including 75 for Azadi ka Amrit Mahotsav, across 33 states/UTs. The first round (February-April 2022) involves 11 states, while the remaining 22 states will conduct rounds from April-May 2022. Overall, ten phases of Mission Indradhanush have covered 701 districts nationwide, with a total of 96.8 lakh pregnant women and 3.86 crore children vaccinated as of April 2021.

  • IMI 5.0
    IMI 5.0 guarantees that routine immunization services extend to children and pregnant women who were previously missed or dropped out nationwide. For the first time, the campaign encompasses all districts and includes children up to five years old. IMI 5.0 endeavors to augment immunization coverage for all vaccines outlined in the National Immunization Schedule under the Universal Immunization Programme.
